The heartbreaking death of 12-year-old Gavin Peterson has shaken the Utah community, as investigators revealed years of severe abuse and neglect.
Found unresponsive on the bathroom floor of his father’s home in July 2024, Gavin’s death has drawn attention to numerous failures in the system that was meant to protect him.

On July 9, 2024, deputies discovered Gavin malnourished and not breathing.
Despite attempts to revive him and rushing him to a hospital, doctors were unable to save him.
His father, Shane Peterson, stepmother Nichole Scott, and older brother Tyler Peterson were arrested shortly after.

Missed Opportunities by Child Protective Services
Recent revelations from Utah’s Division of Child and Family Services (DCFS) have raised serious questions about missed intervention opportunities.
Since Gavin was an infant, welfare concerns had been raised, but no substantial action was taken.

The abuse appeared calculated and continuous, as conversations detailed restricting his movements and rationing even the smallest amounts of food and water.

Gavin’s biological mother, Melanie Peterson, who lost custody of her children after a drug-related charge, had made repeated attempts to alert authorities to the abuse.
Since regaining visitation rights in 2019, she had seen signs of malnourishment and attempted to report the conditions her son was living under, but little was done to address her concerns.
Community’s Growing Concern
Staff at Gavin’s elementary school had also noticed troubling signs.
A cafeteria worker revealed that she and a colleague would often sneak Gavin food, despite the school’s reluctance to provide him with meals due to bureaucratic policies.
These acts of kindness were, unfortunately, not enough to save him from the horrific abuse he faced daily.
Arrests and Future Legal Proceedings
Following their arrests, Shane and Nichole were charged with child abuse homicide and several other serious crimes.
Tyler, similarly, faces charges for his involvement in the prolonged abuse.
